Diabetes Management

Cycle of Care Diabetes Programme

​

We are registered with the HSE for the Diabetes Cycle of Care Programme. This programme is available free of charge to all patients with a diagnosis of type 2 Diabetes who hold a valid medical card or a GP visit card under the age of 70. The patient will be scheduled for 2 visits in the year. 

 

During these visits relevant blood tests will be taken, a review of your diabetes including relevant examinations will be performed, your medication will be reviewed, education in certain areas relating to your diabetes will be discussed and any relevant referrals will be made.

 

Patients who do not hold a medical card or a GP visit card

For Private patients who are not able to register for this programme we undertake a similar programme.

 

Diabetic Retina Screening

​

All patients public and private with Type 1 & 2 Diabetes are entitled to a free retina exam yearly to screen for early eye problems related to your diabetes. For further information and to register please click on the following link: https://www.diabeticretinascreen.ie/screening.10.html#Who

 

Long Term Illness Scheme

​

All patients  public and private who have been diagnosed with Type 1 or Type 2 Diabetes are entitled to free medications related to the illness by applying for a Long Term Illness Card. For further information please click here

Chronic Disease Management

The HSE introduced a new programme in 2020 to improve the care of patients with certain chronic illnesses;

​

  • Diabetes Type 2

  • Asthma

  • Chronic Obstructive Pulmonary Disease (COPD)

  • Cardiovascular Disease including:

​

- Heart Failure

- Ischaemic Heart Disease

- Cerebrovascular Disease (Stroke/ Transient Ischemic Attack (TIA))

- Atrial Fibrillation

If you have a medical card or a GP visit card, are over 70 and have one or more of the listed chronic diseases you are eligible to be registered for this programme.

​

The programme will involve 2 to 4 visits over the year to your doctor and nurse.

 

During these visits blood tests relating to your disease or treatment will be taken, a review of your condition including relevant examinations will be performed, your medication will be reviewed, education in certain areas relating to your illnesss will be discussed and any relevant referrals will be made.
